Interlibrary Services Request Form

What is Document Delivery?

The Interlibrary Services staff mail materials that are available in the library collections in the Darling, Marshburn, and Stamps libraries located on the Azusa campus. This service is available to students, faculty and staff at Azusa Pacific University's Regional Centers. The Interlibrary Services staff can send:

  • Books or chapters from a book
  • Copies of articles in periodicals
  • Education test kits
  • Videos or DVDs

How do I request material?

The Interlibrary Services Request form is available online. The form must be completed correctly or it will not be processed. Requests by phone or e-mail will not be accepted.

Is this service free?

There is no charge to request books, media items, or test kits, and return postage is included for returning these items to the Azusa campus.

Photocopies of articles cost $2.00 for the first 20 pages and $0.10 each additional page. Fax and rush orders incur an additional $3.00 charge. The library will send a statement.

How long does it take to receive materials?

Items will be mailed, faxed, or emailed within 2 business days of receipt of your request.

How long may I keep the material?

You may keep copies of articles. Books are generally loaned for 4 weeks and videos for 2 weeks. Test loan periods are determined by the Library Media Center. Items may be renewed at the regional centers. Overdue material will incur fines and blocks on the patron record.

Where do I return books, videos or tests?

Return all items by mail using the postage-paid mailing label included in your package.
